Degrees and DiplomasDiploma Name VerificationThe Office of the Registrar maintains the official name and address for all students. Your name will appear on the diploma in the same format as it is stored in the Office of the Registrar records (generally, first name, middle name, last name). In some cases, students prefer to have their full middle or maiden names included on their diploma. It is your responsibility to verify that the name listed is correct, or to provide any changes required, including hyphens, spaces, lower/upper case letters, accents, etc. The IUPUI Office of the Registrar will process duplicate diplomas requests for degrees awarded at the IUPUI campus.
Name ChangeThe Office of the Registrar maintains the official name and address for all students. Students whose names have been legally changed or require editing should file a Request for Change of Name form. Legal documentation, such as a copy of a social security card, marriage certificate, or official court document, must be provided to support the name change. This name change will be reflected on the student’s official academic record.Please do not E-Mail your information, your hand written signature is required.
If you are or have ever been employed by Indiana University, even as a work-study employee, please provide documentation, such as a photocopy of your driver’s license. If you are or were an IU employee and your marital status has changed, please provide a copy of appropriate documentation. This will be used in adjusting any employee benefits.
